Is this correct? And do I need to provide any documentation as to why I am not providing this information? (ex. Do I need to write anything on Attachment D Offeror's References)
Current vendors who have been awarded contracts are not required to provide monitoring reports for the last 24 months. Also, they are not required to submit references. (Feel free, but not required, to notate on a separate page why the documentation was not provided)
The “Staff Qualification Statement” located within the SOW does not need to be filled out at the time of submission. That form is to be used after a contract is awarded and the Vendor add new staff under the agreement.
The “Change or Addition of Performance Site” located within the SOW does not need to be filled out at the time of submission. That form is to be used after a contract is awarded and the Vendor chooses to relocate or add an additional site within the catchment area.
Section M is the evaluation Factors for Award and is for US Probation use.
